A Survey

I created and proposed using the survey shown below when the college was first put on probation. When a problem is reported, it should be important to know how big a problem it is. Unfortunately it seems too many college Administrators don't want to know the extent of the problem. 

The intent of the survey below is to determine in six month intervals if the culture is changing -- by examining the actions of the people who make up the culture. If the number of negative incidents goes down, then the culture is changing for the better. If the number of negative incidents goes up or stays the same, then some changes probably need to occur. 

Of course the number of negative incidents will never reach zero, but that's OK. We're looking for trends in the numbers as well as problem areas that need to be addressed. For example, if negative incidents happen every year at out-of-town conferences, then a pre-conference meeting needs to happen -- with the promise of a post-conference meeting to let everyone know they should be on their best behavior. The important thing is to gather evidence to make decisions on what needs to be done next based on the evidence and not on the administration's guesses and wishes.
